Nutrition habits and physical activity becomes more, more important to adults, but difficult to adjust, in adulthood. Coaches should pause and encourage consumers to take a moment and consider the changes that have been made and the impact.

Quick Tips:

1. Maintain communication with clients by sending out this survey periodically throughout your time together.
2. You may choose to send this survey electronically or use the questions as conversation prompts to discuss out loud.
3. Some clients may feel more comfortable bringing up sensitive concerns in writing rather than talking about them.
